Employee Wage Claim Form (MW-31A):
Claim for wages due an employee working in New Jersey or for a New Jersey based company.
Please print legibly and complete all information. Give particular attention to the questions requesting the proper trade name (the name on the business cards) and the corporate name. Also, supply the proper street number and complete address. Be sure to supply a New Jersey address for your employer or the New Jersey location where the work was performed. If the company is no longer in business, please supply the home address of the employer, or any information concerning the employer.

Employee Wage Claim Form for Non-Payment of Prevailing Wage Rate (MW-31B)
Please print legibly and complete all information. Give particular attention to the questions requesting the proper trade name (the name on the business cards) and the corporate name. Also, supply the proper street number and complete address. Be sure to supply a New Jersey address for your employer or the location where the work was performed. If the company is no longer in business, please supply the home address of the employer, or any information concerning the employer.
Please indicate by using a separate piece of paper, if necessary:
  1. the rate of pay for each trade/craft including fringe benefits;
  2. the trade/craft performed;
  3. the location where the work was performed, including all sites.
Employee Mandatory Overtime Complaint Form (MW-31OT):
The New Jersey Wage and Hour Law stipulates the conditions under which overtime may be required for certain employees of health care facilities. Employees who are covered by the law are involved in direct patient care activities or clinical services and receive an hourly wage (physicians are not included). A “health care facility” means a health care facility licensed by the Department of Health and Senior Services, a State or county psychiatric hospital, a State developmental center, or a health care service firm registered by the Division of Consumer Affairs in the Department of Law and Public Safety.
An employee who meets the above criteria and has been required to work overtime may file a complaint if he/she believes the overtime was not in response to an unforeseen emergent circumstance, and/or the employer did not exhaust reasonable efforts to obtain staffing, and/or the employer did not provide the employee with the allowed time to make arrangements for the care of family members.

Apparel Industry Certificate of Registration - Employer Application (MW-56):
The New Jersey Apparel Industry Registration Act, requires the registration of anyone engaged in manufacturing or contracting in the apparel or embroidery industries in New Jersey as well as any person outside the State who contracts to perform work in the State of New Jersey.
*Failure to complete Question19 may result in delay or denial of your application.

This application must be accompanied by a check or money order, made payable to the Commissioner of Labor and Workforce Development in the amount of $300.
Out of State Permit – Application for a Permit to Maintain Payroll Records Outside of the State of New Jersey (MW-42)
Any out of state employer that does not maintain its payroll records within the State of NJ must apply for a permit authorizing the employer to keep payroll records outside the State of New Jersey.
Payroll Certification for Public Works Projects (for Contractor and Sub-Contractor's Use for Weekly and Final Certification):
Effective February 18, 1992 Regulation N.J.A.C. 12:60-2.1 and 6.1 of the New Jersey Prevailing Wage Act, N.J.S.A. 34:11-56.25 et. seq. requires that certified payroll records must be submitted to the public body for each employee on the project. The General Contractor is responsible for ensuring that each sub-contractor submits the certified payroll within ten (10) days of the payment of wages. The public body shall receive, file and make available for inspection during normal business hours the certified payroll records.
