Disability During Unemployment (DDU)

You may qualify for disability benefits during unemployment if you are unemployed and unable to work due to illness or injury not caused by your job. If you already filed a regular unemployment claim, the disability claim will be based on that claim.

Get the "Claim for Disability Benefits" (Form DS-1) by calling or writing your employer, union, One-Stop Career Center or the Division of Temporary Disability Insurance, PO Box 387, Trenton, NJ 08625-0387 (telephone 609-292-7060).  See DISABILITY INSURANCE PROGRAMS for additional information about applying for temporary disability benefits and to download a claim form directly from their website.
