Change of Address - State Plan

If you move or change your mailing address during your period of disability, you must notify this office of your new address in writing. For security reasons, we can not change your mailing address by telephone. Please be sure that your correspondence includes your complete new address, your signature and your Social Security number. If you have received forms C05, Notice to Claimant of Receipt of Claim for Disability Benefits,  it may be used to notify our office of a change or correction.

Remember:  If you have received a debit card, you must also notify Bank of America of your new address.  The bank can be contacted through their website at www.bankofamerica.com/cashpay or by telephone toll free at 1-866-213-4074.
