General Information - This page contains general information on the Disability During Unemployment Program.

Download DDU Forms - This page contains links to forms that are mailed by the Disability During Unemployment Office with benefit checks.  The Notification of Information Change -DDU and the Claim for Continued Benefits - DDU are available for download. 

ELIGIBILITY REQUIREMENTS

Wage Requirements - An individual must meet certain wage requirements to be eligible for temporary disability benefits under the Disability During Unemployment Program.  This page provides the latest information on the minimum number of weeks worked and the minimum amount that must be earned to have a valid claim.

Dependency Allowance - If you do not quallify for the maximum weekly benefit rate under the Disability During Unemployment Program, you may be entitled to a dependency allowance.  This page explains dependency benefits and defines who is considered a dependent.

Disqualifications - This page lists conditions that cause benefit disqualifications for a Disability During Unemployment claim.

Limitation of Benefits - This page lists the conditions that cause Disability During Unemployment Benefits not to be paid.
