GENERAL INFORMATION

Changing Your Address - This page explains how to change an address, the use of multiple addresses and having a disability address.  A link is provided to the Division of Employer Accounts for tax file address questions.  Telephone numbers are provided for the State Plan Employer Charge Unit and Claims Control Section of Unemployment.  A link to download a form to change the disability mailing address is provided.

Coverage for Temporary Disability Benefits - This page provides basic information on covering workers for temporary disability benefits.

Worksite Visits/Presentations - Temporary Disability Insurance staff are available for worksite visits and/or presentations on the Temporary Disability Benefits Program.  This page explains what is offered and how to request a visit.  You may send your request via the internet directly from this page.


ELIGIBILITY REQUIREMENTS

Wage Requirements - An individual must meet certain wage requirements to be eligible for temporary disability benefits.  This page provides the latest information on the minimum number of weeks worked and the minimum amount that must be earned to have a valid claim.  The page also includes the definition of a "base week" and "base year".

Limitation of Benefits - In addition to wage and medical requirements, an individual must meet certain eligibility criteria.  This page provides a list of limitations which impact an individual's eligibility to receive benefits or may reduce the amount of benefits payable on a disability claim.

Nonduplication of Benefits - In certain situations, the Temporary Disability Benefits Law requires that benefits be prohibited, or reduced because an individual is receiving some type of benefits for the same period of disability.  This page lists the types of situations which may be a bar to, or reduce benefits.

Continued Pay - As an employer, you may choose to continue paying your employee during their period of disability.  This page explains continuation pay and how it impacts the payment of disability benefits.


APPLYING FOR BENEFITS

How to Apply - This page describes how a worker applies for temporary disability benefits.  It includes links to download a claim form or order forms and pamphlets.

Benefit Calculations - Temporary disability benefits may be payable up to twenty-six weeks for any one period of disability.  This page explains how we calculate both the weekly benefit rate, and the maximum amount payable on a claim.  The page also includes information on the current maximum weekly benefit rate which changes January 1 of each year.

Submitting Wage Information - This page provides information and explains your responsibilities in reporting wage information when an individual applies for temporary disability benefits.


AFTER APPLYING

Appeals - Go to this page to learn more about the appeal process including how to file an appeal, and the timeframe for appealing a decision.  If you choose to file your appeal via the internet, you may do so directly from this page.

Charge Notices (Form DS-7C) - The employer for whom the claimant last worked immediately prior to the onset of the disability assumes all the charges for all benefits paid to the claimant for that period of disability.

Independent Medical Examinations - During the course of the disability , you may request your employee be scheduled for a medical examination by a State-appointed physician.  This page provides information about the process and explains how to request  the exam.  A link is provided to make your request directly from the website.

Tax Information - This page explains the employer's tax responsibility, deductions from payments and taxable portion of benefits.  It also gives information about reporting benefits on form W-2.
