Family Leave Insurance Web Services

Web Inquiry If you have filed a claim for State Plan Temporary Disability or Family Leave Insurance you will be able to see and print some basic claim information for any claim you filed in the past 1,000 days. Available information includes:

  • Claim Status
  • Entitlement Details
  • Payment History
  • Payment Status

Information is up-dated daily and is available from 7am to 6pm, Monday through Friday, including holidays.

Additional information regarding your eligibility and payment status can also be found on the Notice of Determination, check stubs and other documents mailed to you. You can also contact our Customer Service Section at (609) 292-7060 during regular business hours - 8:30 am to 4:30 pm or by writing to our office at Division of Temporary Disability Insurance, PO Box 387, Trenton, NJ 08625-0387.

File A Transitional Bonding Claim
Use this application only if you have received State Plan Temporary Disability Insurance Benefits and have received Form FL-2, Family Leave Insurance Benefits - New Mother Bonding. Do not file your Family Leave Insurance claim until you have recovered from your pregnancy related disability.

You can file your claim from 7am to 6pm, Monday through Friday, including holidays.
