Change of Employer Information - To change your mailing address, trade name or corporate name, please complete NJ-REG-C Please include your New Jersey Taxpayer ID number, the name of your organization, your old address and new address, and, if applicable, your new trade name or new name of your corporation.
Special note: If you change the name of your corporation, you must attach an amended certificate of incorporation to your letter.

Inactivating Your Account - To make your account inactive please submit a short letter stating such, your New Jersey Taxpayer ID number and the exact date of your last payroll in New Jersey.
