URBAN ENTERPRISE ZONE PROGRAM

An employer who has been certified by the New Jersey Department of Community Affairs may be eligible for a rebate of unemployment contributions paid on wages to qualified employees under the New Jersey Urban Enterprise Zone (UEZ) Program.

If you believe that you are eligible for this award, please print and complete the "Annual Calculation of Employer Unemployment Rebate," FS-131, and the "Employee Certification," FS-131A for the calendar year 2012. The completed from must be signed with an authorized signature and mailed by Friday, May 24, 2013 to:

After the application has been reviewed, your entitlement and the amount due will be certified for payment which is processed by the New Jersey Department of Community Affairs.

In order to expedite the processing of the award, please ensure the business name, address and federal identification number appear as they are listed with the New Jersey Department of Labor and Workforce Development.
