To file an application for disability benefits an individual must visit a local Social Security Office or call the Social Security Administration’s toll-free number 1(800)772-1213. They will ask for information from you. It is advisable to have the following information available:

  • Social Security Number
  • Birth Certificate
  • Details of medical history, including the names and addresses of any doctors or hospitals where treatment was received, the dates of treatment and any tests done.

The Social Security Administration also has the basic disability application and a guide on how to complete it available on line.
